Solvis Home Healthcare, a unit of Solvis Healthcare, Inc., provides home health care personnel and related services to individuals in their homes.  Since 1995, we have been providing 24/7 care to the aged, handicapped, and homebound population. Founded and staffed by practicing nurses, our caregivers meet the urgent and growing need for in-home care and more effective care management.

We provide clients with a single source for all their home care needs, including treating clients with Alzeimers and dimentia. We understand that creating a warm, caring environment is an essential ingredient in the well-being and expedient recovery of our patients.

WE FOCUS ON CARE:  Solvis Home Healthcare has an integrated administrative system that significantly reduces paperwork at our Care Centers.  The result: more time is devoted to personally servicing the needs of our clients.  The innovative processes include an integrated scheduling, billing and payroll program; highly personalized client inquiry and intake processes; and telephonic tracking and monitoring systems.  These proprietary tools enable our staff to increase ongoing contact with our clients and ensure that care plans are up-to-date.  In the home, a dedicated team of trained professionals supports our clients - enabling these individuals to maintain their independence.  Our caregivers - our Care Partners - provide nursing and treatment as dictated by physicians and, in the case of seniors, assistance with activities of daily living such as bathing, dressing, going to the toilet, moving throughout the house, feeding, meal preparation, shopping and light housekeeping.  Our services variously include:

WE FOCUS ON SOLUTIONS: Our goal is to empower clients to craft an at-home solution that fits with their unique set of needs and desires.  Solvis Home Healthcare goes one step further by providing Care Coordination services that can manage all aspects of a client's care and represents a meaningful alternative to facility-based living.

Solvis Home Healthcare begins with an online profile, followed by an in-home evaluation by a Solvis Home Healthcare Client Care Coordinator, a trained home-care professional who meets with the patient and the family.  This profiling is part of our holistic approach that addresses the physical, mental, emotional, social and environmental aspects of the patient's living situation.  A comprehensive and personalized plan of care report is then prepared for the client on an individual basis.

Each client and his or her family are directly linked with a Solvis Home Healthcare Care Coordination Team.  This team provides an easliy accessible, single point of contact with Solvis Home Healthcare: monitoring, updating and scheduling all aspects of an individualized plan of care.  The Care Coordination Team offers comprehensive solutions to challenges faced by our senior clients. The results: a dedicated team providing services that bring peace of mind, comfort and the resources required to recover in place, at home.
